What to bring to an appointment
Many insurance plans require prior authorization from your primary care physician before seeking specialty care. Please check with your insurance company or primary care physician before making an appointment. Please Bring: * Your current valid insurance card * Referral authorization from your primary care provider if necessary. * Medical History to include all medications and dosage taken currently. * All previous x-rays, CAT scans, MRI scans, or other test results relating to your medical problem. * Previous medical records relating to current medical problem.
Telephone
If you have a question or problem that needs the attention of an orthopedic surgeon please call during office hours. It is not always practical to interrupt the doctor during patient exams. Please expect to speak to or leave a message with a physician assistant or staff member. Your call will be returned as soon as possible, usually by the close of office hours. EMERGENCY CALLS WILL BE HANDLED IMMEDIATELY.
Prescriptions and Refiils
Prescriptions and refills are issued during weekday office hours only. For a prescription refill, call the pharmacy first and they will contact the office. Remember to check your medicine requirements prior to weekends and holidays. We require 24-hour notification on all refills. The physician on call will not refill narcotic prescriptions.
Fee and Billing Procedure
While all fees are due and payable at the time of service, as a courtesy to patients we bill most health insurance carriers, provided we have sufficient information. However, you retain financial responsibility for all charges incurred and will receive a monthly billing statement. We do not bill 3rd party carriers and require payment at the time of your appointment for services provided under these plans. Payment for services rendered is the responsibility of the patients regardless of insurance or outside arrangements such as legal litigation. If you have financial difficulties, please notify the office regarding fees. There is a nominal fee for the completion of disability insurance forms and attorney consultations.
X-RAYS
The courts have established that x-rays are the property of the doctor who has taken them and that they are to be retained in his possession as a part of the patients medical record. X-rays may be burned onto a CD, downstairs at the South Florida Diagnostic Center. There is a fee for copying x-rays.
